When connected, check in which manner is set the phone to be connected - Storage, MTP, Mirroring or whatever (can't recall them all). Change to the option it works as expected (in Pie it is Mirroring, of course).

Does the phone need to be connected via the USB for all functions to work?

Yes, it must be. With proper cable. Many times there is an issue with false USB cable. When connected trough AA USB is for data (screen mirror, apps, music) and automaticaly pair phone via BT for voice.

Replaced R-Link2 V2.2 SW version 9. 0.34.611, enabled Android Auto and CarPlay (change USB slots)

This is a slightly different issue though. AA and Google voice assistant work perfectly when connected via USB. However, now when I'm not using USB or AA the car's inbuilt voice recognition doesn't work. It now says that I need to connect a compatible smartphone via Bluetooth....but the phone is connected as I can still make and receive calls. Very odd.
